Applicants must be:

A citizen of the United States
18-62 years of age
In reasonably good health

(1) Enlisted Packet	Contains all Enlisted forms and instructions
(2) Officer Packet	Contains all Officer forms and instructions
(3) - Enlisted NYG Form 650	Recommendation for enlistment in the New York Guard (Completed by Unit)
(3) - Officers Letters of Recommendation	Two letters of recommendation
(4) NYG Form 611	Agreement of Understanding - Complete all items
(5) NYG Form 615	Authorization for release of information - Complete all items
(6) NYG Form 620	Personal History - Complete all items
(7) NYG Form 620-A	Personal History Questionnaire continuation sheet
(8) NYG Form 625	Service Record. For initial entry into the NYG, complete items: 1, 2, 3 and 4. If applicant has prior military service, complete all items that are applicable
(9) NYG Form 6130.1	Report of Medical Examination
(10) NYG Form 6130.2	Report of Medical History
(11) NYG Form 660	Oath of enlistment/re-enlistment. Completed by unit. With written approval from Headquarters, New York Guard administer oath of enlistment/re-enlistment and submit original to Headquarters, New York Guard
